Description Identifies systemic therapeutic procedures administered as part of the first course of treatment at this and all other facilities. If none of these procedures were administered, then this item records the reason they were not performed. These include bone marrow transplants, stem cell harvests, surgical and or radiation endocrine therapy. Rationale This data item allows the evaluation of patterns of treatment which involve the alteration of the immune system or change the patient's response to tumor cells but does not involve the administration of antineoplastic agents. In addition, when evaluating the quality of care, it is useful to know the reason if these procedures were not performed. Instructions for Coding Bone marrow transplants should be coded as either autologous bone marrow originally taken from the patient ; or allogeneic bone marrow donated by a person other than the patient ; . For cases in which the bone marrow transplant was syngeneic transplanted marrow from an identical twin ; , the item is coded as allogeneic. Stem cell harvests involve the collection of immature blood cells from the patient and the reintroduction by transfusion of the harvested cells following chemotherapy or radiation therapy. Endocrine irradiation and or endocrine surgery are procedures which suppress the naturally occurring hormonal activity of the patient and thus alter or effect the long-term control of the cancer's growth. These procedures must be bilateral to qualify as endocrine surgery or endocrine radiation. If only one gland is intact at the start of treatment, surgery and or radiation to that remaining gland qualifies as endocrine surgery or endocrine radiation. Code 00 if a transplant or endocrine procedure was not administered to the patient, and it is known that these procedures are not usually administered for this type and stage of cancer. Code 00 if the treatment plan offered multiple options, and the patient selected treatment that did not include a transplant or endocrine procedure. A meso scale flow reactor for automated reaction screening and optimization using minimum amounts of material is presented. Flowing reaction segments each corresponding to different compositions, residence times, or temperatures are cleanly separated from one another by helium gas spacers. Multiple segments can be flowed through the system simultaneously allowing for parallel-sequential operation. The system is composed of four types of modules: 1 ; the Feed Module controls the primary system flow; 2 ; the Fluid Preparation Module prepares 200-400 L reaction segments by measuring and mixing up to four components; 3 ; the Interface Module conditions the reaction segments and injects them into the primary flow; and 4 ; the Reactor Module controls the reaction temperature, pressure and residence time. The output from the Reactor Module can be connected to analytical devices, sample collectors, or merged with other reaction streams. The flow rate for mixing components within the Fluid Preparation Module is independent of the primary flow from the Feed Module, thus allowing fast mixing in simple tubing under near turbulent conditions with a fast mixing flow rate and long residence times with a slow primary flow rate. Copper is used in the Reactor Module for excellent thermal control. Inline optical detectors allow tracing segment locations and calibrating flow rates. The data presented show square wave-shaped segmented flow, a linear response from metered segment components, quantified mixing rates, and aldol chemistry. The overall system allows for 100-300 reactions per day depending on residence time for reaction screening, optimization, and or Medicinal compound library synthesis.

Psychiatric disorders and difficulties in the social, educational, and relationship life domains of children and adolescents. The aim of this study was to determine the effects of alexithymia on pain without an organic cause and psychiatric symptoms in children and adolescents. Methods: The study included 15 patients with depression, 21 patients with complaints of pain without an organic cause, and 15 controls with neither a current or previous psychiatric disorder, nor complaints of any kind of pain. All the participants were evaluated and compared for alexithymia and psychiatric symptoms rated by the Toronto Alexithymia Scale TAS ; , Beck Depression Inventory BDI ; , and State-Trait Anxiety Inventory for Children STAIC ; . Results: Although the lowest mean TAS score was in the control group 9.3 2.8 ; and the highest score was in the pain group 11.2 4.0 ; , there was no statistically significant difference between the groups P 0.05 ; . Adjustment ability and self-perception are both influenced by the severity of the disease. The capacity of using mental processes is closely related to the quality of problem solving behaviors. The presence of problems in executive functioning, language ability, and visual-spatial perception is frequently indicated in the literature for children with ADHD. In a normal developmental process, children progress to abstract thinking by the age of 12 years and have the ability of justice in the assessment of the events. Findings in the literature related to the judgment ability of ADHD children are inconsistent and it is not clear if it is similar to or different than normal children. Problems in the assessment of events cause problems with the reactions to these events. This condition then causes problems in self-perception and necessitates adult supervision. In this regard, parental behavior is important in ADHD children. This study included 39 children between the ages of 6 and 11 years mean: 8.2 1.3 ; with the diagnosis of ADHD according to DSM-IV diagnostic criteria, and their mothers. The Wechsler Intelligence Scale for Children-Revised WISC-R ; and PiersHarris Children's Self-Concept Scale were administered to children and the Problem Solving Inventory was administered to the mothers. Among the ADHD children, 20 were predominantly inattentive, 13 were hyperactive, and 6 were combined, according the subtypes of ADHD. Mean scores of WISC-R were as follows: verbal IQ score: 101.36 9.07; performance IQ score: 103.66 10.7; total score: 102.19 6.3. They have been described as the `crme de la crme' of students. It is, therefore, generally expected that they perform very well in medical school. It is not unusual, however, for some of these brilliant students to fail badly in their examinations or to be advised to withdraw from medical school because of poor academic performance. On the basis of our experience as medical students, medical practitioners, and now, medical teachers, we have attempted an analysis of the factors that contribute to this gloomy scenario. Information course-content ; overloading has been identified as a major contributing factor. This derives from the amount of information that the average medical school teacher pushes down the throat of the students. Much is unnecessary for the level of training, and if we must drop the `excess luggage' to enhance the flight of the students through medical school, deliberate policies in that direction must be formulated and implemented. Course content overloading is expected to get worse, with the ever-present technology-induced exponential growth in information. Cognizant of this fact, several regulatory bodies like the Association of American Medical Colleges AAMC ; and the General Medical Council GMC ; of Britain have developed and promoted the concept of a core curriculum for each of the principal disciplines in Medicine. Several medical schools have adopted this concept and, also, the problem-based learning PBL ; approach which focuses on ameliorating the complexities associated with information growth in medical education. We have discussed the factors that promote `excess luggage' from the perspective of the curriculum, medical students and the textbooks, the changing quality of the pharmacology teacher and his psychology, psychology of the medical student, and the dearth and role of the medically qualified pharmacologist. It is concluded that any medical school worth its name should have a well established medical education unit that promotes continuing teacher training for academic staff, awareness of the issues surrounding `excess luggage', including the concept of the core curriculum, and ensures a periodic review of the curriculum.

Using standard in vitro assays and isolates that contain ESBLs may appear susceptible to 3rd-generation cephs or aztreonam. The difficulty arises from the fact that breakpoint panels from commercial automated susceptibility systems currently use an MIC breakpoint for ceftazidime, ceftriaxone, cefotaxime, and aztreonam that is 8 g ml. Some strains that express ESBLs have MICs to the 3rd-generation cephs and aztreonam as low as 2 g ml. Therefore, a patient may be initially treated with a 3rd-generation ceph based on laboratory results that suggest the organism is susceptible. Two different investigators, using genetically defined and well characterized ESBL isolates, recently found that using cefpodkxime at an MIC breakpoint of 2 g ml, accurately detected all strains expressing ESBLs. Commercial automated susceptibility systems already have, or will have in the near future, cefpodoxime on their panels cards to help identify strains that contain ESBLs. Our laboratory is currently defining an ESBL producing isolate as any Enterobacteriaceae that is resistant or intermediate to either ceftazidime or ceftriaxone or cefotaxime or aztreonam, yet susceptible to cefoxitin and cefotetan. We confirm each case using a double-disk diffusion test3 , which tests the enzymes susceptibility to lactamase inhibitors such as clavulanate. In contrast, ampC mutants are resistant to both cefoxitin and cefotetan and are resistant to the inhibitory action of -lactamase inhibitors. Our protocols for monitoring and testing for ESBLs are available to clinical laboratories. The NPHL is interested in monitoring the frequency of ESBLs throughout Nebraska and isolates may be referred for confirmatory testing or molecular typing. Contact Dr. Paul Fey for more information.
